Primary and secondary lymphoedema

Recognise lymphoedema, distinguish developmental primary disease from acquired secondary causes, exclude dangerous mimics and build a safe long-term management plan.

Acute change in a swollen limb

Rapid new swelling, severe pain, spreading erythema, systemic illness, cyanosis or neurovascular compromise is not explained by stable chronic lymphoedema.

Action: Assess urgently for DVT, cellulitis or sepsis, acute arterial compromise, compartment syndrome and obstructing malignancy; suspend routine compression when the acute diagnosis or perfusion makes it unsafe.

Lymphoedema is chronic swelling caused by inadequate lymphatic drainage relative to tissue fluid load. Protein-rich interstitial fluid promotes inflammation, adipose deposition and fibrosis, so an initially soft pitting limb can become firm, enlarged and structurally distorted. Primary lymphoedema results from developmental or genetic abnormalities of lymphatic vessels or nodes. The abnormality exists from birth, although swelling may present in infancy, around puberty, during pregnancy or much later after an additional stress. Secondary lymphoedema follows acquired injury or sustained overload: lymph-node surgery or radiotherapy, cancer, trauma, recurrent infection, venous disease, severe obesity, immobility and inflammatory disease are common UK contexts. The primary-secondary label describes cause, not severity, and mixed mechanisms are frequent.

Assessment establishes chronology, distribution, tissue characteristics, functional effect, risk factors and alternative diagnoses. Compare both limbs; inspect toes, skin folds and scars; palpate pulses; document pitting, Stemmer sign, fibrosis and skin change; and use consistent circumference or volume measurements. The most important test is often a carefully constructed history rather than a scan. Investigate heart, renal, hepatic, venous, endocrine or malignant disease when the pattern suggests systemic or proximal obstruction. Lymphoscintigraphy, indocyanine-green lymphography, MRI or ultrasound can answer selected anatomical questions, but no single result replaces clinical context. Long-term care uses supported self-management and specialist compression to control swelling, preserve function and reduce skin breakdown and cellulitis risk; it rarely removes the underlying susceptibility.

Key points

  • Treat rapid new swelling or systemic illness as a diagnostic change: urgently exclude DVT, cellulitis or sepsis, acute ischaemia and obstructing malignancy before resuming routine care.
  • Primary lymphoedema reflects a developmental lymphatic abnormality present from birth but may become clinically apparent at any age; secondary lymphoedema follows acquired damage or overload.
  • Diagnosis is usually clinical after a full history, examination and repeatable limb measurement; imaging is reserved for uncertainty, proximal obstruction, surgical planning or an atypical course.
  • Early disease is often soft and pitting and may improve with elevation; later fibrosis, adipose deposition, deep folds, hyperkeratosis and papillomatosis make pitting less evident.
  • Foot or hand involvement, squaring of digits and a positive Stemmer sign support lymphoedema, but a negative Stemmer sign does not exclude early or proximal disease.
  • Management combines skin care, movement and exercise, suitable compression, weight management, education and selected lymphatic drainage within an individualised specialist plan.
  • Diuretics do not correct lymphatic transport failure and are not routine lymphoedema treatment; use them only for a separate indication such as heart failure.
  • Compression must be prescribed and fitted by a trained practitioner after checking arterial status, shape, skin, sensation, dexterity and the person's ability to apply and tolerate it.

Developmental lymphatic abnormality

Primary lymphoedema arises from malformed, sparse or functionally abnormal lymphatic vessels or nodes. Genetic syndromes explain some cases, while many remain clinically classified without a molecular diagnosis.

Acquired lymphatic injury

Node dissection, radiotherapy, trauma, infection and tumour can interrupt previously functioning channels. The anatomical drainage territory often predicts the distribution, although onset may be delayed.

Chronic lymphatic overload

Venous hypertension, severe obesity, immobility and inflammatory disease increase filtration or impair pumping until lymphatic reserve fails, producing mixed secondary chronic oedema.

Medicines and systemic load

Some medicines and cardiac, renal or hepatic disease increase peripheral fluid formation. They can expose limited lymphatic reserve but require their own diagnosis rather than a primary label.

03PathophysiologyThe causal sequence from the underlying abnormality to symptoms and harm.

    Transport failure

    Lymphatic uptake or propulsion cannot match capillary filtration, allowing protein-rich fluid and cells to accumulate in interstitial tissues.

    Chronic inflammation

    Persistent macromolecules activate inflammatory pathways and impair local immune surveillance, promoting recurrent infection and progressive tissue remodelling.

    Fibrosis and adipose deposition

    Long-standing inflammation stimulates collagen and fat accumulation, turning a soft pitting swelling into a firm enlarged limb with reduced tissue mobility.

    Pump dependence

    Lymph flow relies partly on intrinsic vessel contraction, breathing and surrounding muscle movement; immobility therefore reduces clearance and worsens dependency-related swelling.

04Clinical features and red flagsSymptoms, examination findings, patterns of presentation and time-critical warnings.
Distribution and onset

Record whether swelling began distally or proximally, affects one or both limbs, and involves the foot, hand, genitalia, trunk, breast, head or neck. Link onset to surgery, radiotherapy, infection, trauma or immobility.

Tissue evolution

Early lymphoedema is usually soft and pits under pressure. With chronic inflammation, fibrosis and adipose deposition make tissues firm, reduce pitting and create distorted contours, deep folds and lobules.

Distal signs

Look for dorsal foot or hand swelling, squared toes or fingers, thickened skin folds and inability to pinch skin at the base of a digit. These support lymphatic failure but are not universally present.

Skin complications

Dry fissured skin, hyperkeratosis, papillomatosis, fungal infection, lymph blisters and lymphorrhoea show barrier failure and advanced disease. Mark warmth or erythema and assess systemic features before calling inflammation chronic.

Primary disease clues

Presentation in childhood or adolescence, family history, bilateral but asymmetric foot-led swelling, syndromic features or genital involvement suggest primary disease, yet adult presentation does not exclude a congenital lymphatic abnormality.

Red flags requiring action

  • A hot painful erythematous limb with fever, rigors, hypotension or confusion suggests cellulitis with possible sepsis and requires prompt antimicrobial and severity assessment.
  • Abrupt unilateral swelling, deep tenderness, new chest pain or breathlessness requires an urgent venous thromboembolism pathway rather than attribution to established lymphoedema.
  • New adult-onset unilateral swelling with weight loss, pelvic or axillary symptoms, lymphadenopathy or rapid progression raises concern for malignancy or proximal obstruction.
  • A cold pale foot, rest pain, tissue loss, absent pulses or new neurological deficit requires urgent arterial assessment before compression is intensified.

Investigation order

Read from the initial assessment onwards. Tests may run in parallel in urgent care; first-line, preferred, confirmatory, definitive and gold-standard labels appear only when the chapter explicitly states them.

    Why
    Establish distribution, stage, skin and tissue change, functional burden and a repeatable baseline for follow-up.
    Interpretation and limitations
    Persistent swelling with characteristic distal and tissue findings supports lymphoedema. A percentage volume difference helps quantify unilateral disease but bilateral disease needs absolute trends and clinical comparison.

    Arterial and venous vascular assessment
    Why
    Check pulses and use ABPI or toe pressure when arterial disease is suspected; use venous duplex when DVT, reflux or obstruction is possible.
    Interpretation and limitations
    Arterial impairment changes compression safety. Acute thrombosis needs its own pathway, while chronic venous reflux may coexist and create a mixed venous-lymphatic oedema.

    Targeted blood and systemic assessment
    Why
    Investigate cardiac, renal, hepatic, thyroid, inflammatory or nutritional disease when bilateral or generalised oedema and systemic features are present.
    Interpretation and limitations
    An abnormal systemic result may explain increased fluid load but does not exclude established lymphatic failure; treatment should address both the driver and limb consequences.

    Imaging for proximal obstruction
    Why
    Use appropriate ultrasound, CT or MRI when adult-onset unilateral swelling, nodes, pelvic symptoms, cancer history or rapid progression suggests a mass or central venous obstruction.
    Interpretation and limitations
    A proximal lesion requires causal management. A normal local limb examination does not rule out pelvic, abdominal or axillary obstruction.

    Specialist lymphatic imaging
    Why
    Use lymphoscintigraphy, indocyanine-green lymphography or MR lymphography when diagnosis remains uncertain or detailed anatomy is needed for surgery.
    Interpretation and limitations
    Delayed transport, dermal backflow or abnormal channels support lymphatic dysfunction, but modality availability and field of view differ and findings must match the phenotype.

Deep vein thrombosis

Acute unilateral pain and swelling, especially with provoking factors, requires a formal VTE assessment even when chronic lymphoedema is already present.

Chronic venous disease

Gaiter pigmentation, eczema, varicosities and venous ulceration favour venous hypertension, although foot involvement and fibrosis may develop when lymphatic failure becomes mixed.

Systemic oedema

Heart, renal, hepatic or nutritional disease usually produces bilateral dependent swelling with other systemic findings, but asymmetric pre-existing lymphatic damage can distort the pattern.

Lipoedema

Symmetrical painful disproportionate adipose enlargement usually spares feet, retains a negative Stemmer sign and affects women; coexisting obesity or lymphoedema can blur these distinctions.

Proximal obstruction

Pelvic, abdominal or axillary tumour and central venous obstruction can cause rapid unilateral or regional swelling, collateral veins and constitutional or local compressive symptoms.

  4. 4Classify the likely cause as primary, secondary or mixed, assign clinical stage, and refer atypical, progressive, paediatric or complex cases to a specialist lymphoedema service.
02Conservative pathwayRoutine individualised lymphoedema controlUse for routine long-term care after acute infection, arterial compromise and untreated major systemic causes have been addressed.
  1. 1Teach daily washing, thorough drying, bland emollient use, prompt care of cuts and fungal infection, and early recognition of cellulitis.
  2. 2Prescribe the appropriate compression garment, wrap or bandaging through a trained practitioner, confirming fit, arterial safety and the person's application technique.
  3. 3Combine compression with comfortable movement, exercise, breathing, weight support and elevation for symptoms; add specialist lymphatic drainage when it serves a defined goal.
  4. 4Set measurable goals for volume, skin, function, comfort and self-management, then review before the garment loses fit or the clinical pattern changes.
03Escalation pathwayPoor control or complex anatomyEscalationUse when swelling progresses despite adherence, compression cannot be fitted, recurrent cellulitis continues or reconstructive surgery is considered.
  1. 1Recheck the diagnosis, arterial and venous status, proximal obstruction, infection, weight change, garment fit and practical barriers before labelling conservative treatment a failure.
  2. 2Use an intensive decongestive programme with multilayer bandaging or adjustable wraps when specialist assessment finds a reducible fluid component and safe perfusion.
  3. 3Discuss microsurgical reconstruction or debulking only in a specialist multidisciplinary service after optimised conservative care, clear anatomical assessment and realistic outcome counselling.
08ComplicationsImportant consequences, why they occur and why they matter clinically.

Cellulitis and sepsis

Barrier disruption and impaired local immune clearance predispose to bacterial cellulitis. Each episode can further damage lymphatics and occasionally progresses rapidly to systemic illness.

Skin and lymph leakage

Hyperkeratosis, papillomatosis, fungal infection, deep fissures, ulceration and lymphorrhoea increase care burden and create further portals for infection.

Functional impairment

Limb weight, distorted contours and joint restriction can impair gait, work, self-care and garment use, with secondary musculoskeletal pain and reduced activity.

Psychosocial burden

Visible swelling, clothing difficulty, chronic self-management and recurrent infection affect body image, relationships, employment and mental health and should be assessed directly.

09Monitoring and follow-upTreatment response, safety checks and longer-term review.
  • Measure the same anatomical points with the same method and assess tissue texture, pitting, skin folds and regional involvement rather than relying on body weight alone.
  • Review garment fit, pressure marks, pain, numbness, colour change, donning ability and wear pattern; replace or alter compression when size, shape or function changes.
  • Track cellulitis episodes with dates, symptoms, treatment duration and likely portals of entry because apparent recurrences may be inflammatory mimics or one incompletely treated episode.
  • Reconsider malignancy, central obstruction or systemic disease when previously stable swelling accelerates, becomes painful, spreads proximally or is accompanied by constitutional symptoms.

Primary can present late

A developmental lymphatic defect may remain compensated for decades. Infection, pregnancy, injury or weight gain can reveal limited reserve, so adult onset does not automatically mean secondary disease.

Stemmer sign has limits

Inability to lift a skin fold at the toe or finger supports dermal thickening, but early lymphoedema or swelling confined proximally can retain a negative sign.

Mixed oedema is common

Venous hypertension, obesity, immobility and lymphatic failure can reinforce one another. Treating only one mechanism may leave substantial swelling and continued infection risk.

Compression requires skill

More pressure is not automatically better. Arterial supply, neuropathy, limb contour, skin fragility and application ability determine the safest effective garment or bandage system.

11Common pitfallsFrequent interpretation and management errors.
  1. 01

    Calling every chronically swollen leg lymphoedema without checking cardiac, renal, hepatic, venous, medication and malignant causes can miss treatable or urgent disease.

  2. 02

    Using a negative Stemmer sign to exclude lymphoedema is unsafe because early or proximal disease may not thicken the distal skin fold.

  3. 03

    Escalating compression during acute severe pain, suspected ischaemia or undiagnosed rapid swelling can worsen harm and delay the correct emergency pathway.

  4. 04

    Prescribing a diuretic solely to remove lymphoedema fluid ignores protein-rich interstitial disease and exposes the patient to adverse effects without correcting drainage.
