Subclavian steal syndrome

Recognise clinically important subclavian steal, confirm its haemodynamic cause and select medical or revascularisation management safely.

Do not label acute stroke as steal

Persistent focal neurological deficit, inability to walk, dysarthria, diplopia or reduced consciousness is an acute stroke presentation even when subclavian disease is known.

Action: Activate the hyperacute stroke pathway, document last-known-well and obtain urgent brain and vascular imaging; do not delay reperfusion assessment for outpatient steal testing.

A proximal subclavian stenosis or occlusion lowers pressure beyond the lesion. If demand in that arm exceeds direct inflow, blood can travel from the contralateral vertebral and basilar circulation down the ipsilateral vertebral artery to supply the arm. This is the steal phenomenon. It becomes subclavian steal syndrome only when the haemodynamic change plausibly produces vertebrobasilar symptoms or clinically important arm ischaemia. Many people with partial or complete vertebral flow reversal have no neurological symptoms, so an imaging label must be joined to the history rather than treated in isolation.

Typical clues are a lower pressure and weaker pulse in the affected arm, a supraclavicular bruit and arm claudication. Neurological symptoms such as vertigo, diplopia, dysarthria, ataxia, visual disturbance or syncope are most persuasive when reproducibly provoked by use of the affected arm. They remain nonspecific: vestibular disease, orthostatic hypotension, arrhythmia and posterior-circulation TIA can mimic the pattern. A left internal mammary artery coronary graft creates another steal circuit; proximal left subclavian disease may divert coronary graft flow during arm exertion and cause angina even without neurological symptoms.

Duplex couples lesion assessment with vertebral flow direction and can use reactive hyperaemia to expose intermittent reversal. CTA or MRA defines supra-aortic anatomy before intervention or when ultrasound is uncertain. Management always includes atherosclerotic risk reduction. Revascularisation is reserved for a clinical indication such as recurrent TIA or stroke attributed to the lesion, severe or disabling arm ischaemia, coronary-subclavian steal, haemodialysis access dysfunction or selected conduit planning. Endovascular treatment often has lower early morbidity, while open bypass or transposition remains useful for occlusion, unsuitable anatomy, failed endovascular treatment or durability needs.

Atherosclerotic ostial stenosis

Plaque most often affects the proximal subclavian artery, creating a pressure gradient before the vertebral origin and reflecting systemic arterial disease.

Arteritis or radiation injury

Large-vessel vasculitis, prior radiotherapy and less common inflammatory or iatrogenic lesions can narrow the subclavian artery in younger or atypical patients.

Congenital or compressive lesions

Aortic-arch anomalies and thoracic outlet pathology can alter upper-limb inflow, although their anatomical level and physiology differ from classic proximal atherosclerotic steal.

    Proximal pressure loss

    Stenosis before the vertebral origin reduces pressure in the distal subclavian artery and creates a gradient toward the exercising arm.

    Collateral flow reversal

    Blood crosses the vertebrobasilar circulation and travels retrogradely down the ipsilateral vertebral artery to supply tissue beyond the obstruction.

    Demand-related symptoms

    Exercise lowers distal resistance and magnifies diversion, sometimes reducing posterior-circulation perfusion enough to provoke transient neurological symptoms.

    An internal mammary coronary graft or haemodialysis access can increase distal demand and divert blood from myocardium or compromise access performance.

Inter-arm pressure difference

An absolute systolic difference above ten to fifteen millimetres of mercury raises suspicion, especially with an ipsilateral weak pulse or supraclavicular bruit.

Arm-exertional vertebrobasilar symptoms

Reproducible vertigo, diplopia, dysarthria, visual disturbance or ataxia while using the affected arm supports haemodynamic steal.

Upper-limb ischaemiaRed flag

Fatigue, claudication, coolness or delayed exertional recovery can coexist; rest pain and tissue loss indicate more severe arterial compromise.

Coronary-subclavian stealRed flag

Angina triggered by ipsilateral arm exercise in a patient with an internal mammary coronary graft suggests diversion from the coronary conduit.

Asymptomatic steal phenomenon

Reversed vertebral flow found incidentally without compatible symptoms is common enough that it should not be called a syndrome.

Right brachiocephalic disease

Right-sided inflow disease can also reduce ipsilateral common-carotid flow, producing a broader carotid and vertebral neurological pattern.

Red flags requiring action

  • A persistent posterior-circulation deficit or reduced consciousness requires emergency stroke assessment rather than elective vascular investigation.
  • Rest pain, digital cyanosis, tissue loss or a cold pulseless arm indicates severe limb ischaemia and needs urgent vascular review.
  • Angina during left-arm exertion after an internal mammary coronary bypass may be coronary-subclavian steal and needs urgent cardiology and vascular assessment.
  • Syncope without a reproducible arm relationship still requires assessment for arrhythmia, haemorrhage and other dangerous causes.

Investigation order

Read from the initial assessment onwards. Tests may run in parallel in urgent care; first-line, preferred, confirmatory, definitive and gold-standard labels appear only when the chapter explicitly states them.

    Why
    Screen for an inflow pressure gradient and choose a reliable arm for future management.
    Interpretation and limitations
    A difference above ten to fifteen millimetres of mercury supports subclavian disease but is neither sensitive in bilateral disease nor diagnostic of symptomatic steal.

    Why
    Assess lesion velocity, distal waveforms and direction and pattern of vertebral artery flow.
    Interpretation and limitations
    Alternating, bidirectional or retrograde vertebral flow supports progressive steal physiology; correlation with arm-provoked symptoms determines whether it is a syndrome.

    Why
    Unmask latent or intermittent vertebral reversal when resting flow remains antegrade despite strong suspicion.
    Interpretation and limitations
    A cuff-induced increase in arm demand can worsen vertebral flow abnormality, but testing requires a vascular laboratory and careful symptom monitoring.

    Why
    Define subclavian or brachiocephalic stenosis, calcification, arch anatomy and alternative structural causes.
    Interpretation and limitations
    CTA provides procedural anatomy and extravascular detail but uses radiation and iodinated contrast and does not itself prove symptom causality.

    Why
    Provide morphological and flow-sensitive assessment when iodinated contrast or radiation is undesirable.
    Interpretation and limitations
    Flow artefact may exaggerate stenosis, while phase-sensitive information can help distinguish antegrade from retrograde vertebral perfusion.

    Digital subtraction angiography
    Why
    Resolve anatomy and permit treatment when an endovascular procedure has already been selected.
    Interpretation and limitations
    It is invasive and carries embolic, contrast and access risks, so it is not a screening test for nonspecific dizziness.

Posterior-circulation TIA

Spontaneous focal episodes unrelated to arm use may reflect vertebral or basilar embolism and require urgent stroke assessment rather than a benign steal label.

Vestibular disease

Position-related vertigo with auditory or peripheral vestibular findings may mimic steal, but does not explain a reproducible arm pressure gradient and reversed vertebral flow.

Orthostatic or cardiac syncope

Postural hypotension, arrhythmia and structural heart disease commonly cause collapse; pulse, ECG and orthostatic assessment prevent false vascular attribution.

Thoracic outlet syndrome

Dynamic compression can produce arm symptoms and pulse change, often with positional rather than exertional triggers and a more distal anatomical lesion.

Reduces systemic cardiovascular risk associated with atherosclerotic subclavian artery disease.

Atorvastatin

Offer 80 mg orally once daily for established atherosclerotic cardiovascular disease unless interactions, adverse-effect risk or patient preference support a lower dose; review lipid response after two to three months.

Check liver transaminases, interacting medicines, pregnancy potential and muscle symptoms; use a lower starting dose when comorbidity or interaction risk warrants it.

Reduces future occlusive vascular events as part of secondary prevention.

Clopidogrel

When symptomatic atherosclerotic arterial disease supports single-antiplatelet therapy, use 75 mg orally once daily long term, subject to the chosen indication and review.

Review active bleeding, severe hepatic impairment, bruising, blood dyscrasia and CYP2C19 interactions; reversed vertebral flow alone does not mandate treatment.

Vertebrobasilar ischaemia

Severe haemodynamic diversion or associated supra-aortic disease can produce transient or completed posterior-circulation neurological injury and disability.

Critical upper-limb ischaemia

Progressive inflow failure can cause rest pain, digital ulceration, necrosis or impaired healing, particularly when distal arterial disease coexists.

Coronary ischaemia

Proximal left subclavian obstruction can reverse flow through a left internal mammary graft, reducing myocardial perfusion during arm use.

Procedural failure

Endovascular or surgical repair can restenose, occlude, embolise or injure adjacent vessels, so recurrence requires anatomical and clinical reassessment.

09Monitoring and follow-upTreatment response, safety checks and longer-term review.
  • Reassess whether symptoms remain reproducibly linked to arm use and whether their frequency or functional impact is changing.
  • Measure systemic blood pressure from the higher-pressure arm, while periodically checking both arms when bilateral progression is possible.
  • Review hand perfusion, pulses, tissue integrity and any haemodialysis-access or coronary-graft symptoms at each relevant visit.
  • Track smoking status, lipid response, diabetes control, antiplatelet tolerance and other systemic cardiovascular prevention measures.
  • After revascularisation, perform clinical and duplex follow-up according to the repair and intervene only on findings that threaten clinical benefit.
  • Give explicit emergency advice for sustained focal neurology, rest pain, tissue loss or exertional angina.

Phenomenon is not syndrome

Retrograde vertebral flow may be a well-tolerated collateral pathway; a syndrome requires compatible symptoms attributable to the haemodynamic diversion.

Both arms expose the lesion

Using only the low-pressure arm can undertreat systemic hypertension, while using only the high-pressure arm can hide the diagnostic gradient.

Flow reversal has stages

Early steal may show midsystolic deceleration, then alternating flow, before complete retrograde vertebral flow develops with more severe inflow disease.

LIMA changes the territory

With a left internal mammary coronary graft, proximal left subclavian disease can steal from myocardium and present as exertional angina.

Bilateral disease hides asymmetry

Similar pressures in both arms do not exclude severe bilateral subclavian disease, so pulses, waveforms and cross-sectional imaging remain important.

Endovascular evidence has limits

Observational data support good technical success and lower early morbidity, but randomised comparisons with open surgery are lacking.
