Timing of carotid endarterectomy

Choose a time-sensitive carotid endarterectomy plan that preserves early stroke-prevention benefit while recognising neurological and haemorrhagic hazards.

Brain emergency precedes prevention

An evolving neurological deficit, reduced consciousness or new haemorrhage needs acute stroke care before a preventive operation is scheduled.

Action: Activate the stroke pathway, image the brain and intracranial circulation, and involve stroke and vascular specialists immediately; do not let a theatre target delay reperfusion treatment.

01Purpose and principlesWhat the treatment does and how it fits into care.

CEA benefit after symptomatic carotid stenosis is strongly time dependent. A recently active plaque carries a high early recurrent embolic risk, and pooled trial evidence shows the greatest absolute benefit when surgery occurs within 14 days of the last retinal or cerebral event. The clock begins at symptom onset, not at clinic attendance, duplex confirmation or referral. Systems therefore need same-day communication, rapid corroborative imaging and parallel anaesthetic assessment.

Early does not mean unselected emergency surgery. A stable TIA with no infarct may permit very prompt CEA, while an acute stroke can carry infarct swelling and haemorrhagic transformation risk. ESVS recommends deferral when modified Rankin score is at least 3, infarction exceeds one third of the ipsilateral MCA territory, or consciousness is altered. Brain imaging, clinical trajectory and functional prognosis matter alongside stenosis severity.

Thrombolysis and mechanical thrombectomy add separate timing questions. Antithrombotics are withheld for 24 hours after intravenous thrombolysis, with repeat brain imaging before restarting, and ESVS advises considering delay of carotid intervention for six days. Tandem carotid disease treated during thrombectomy follows a hyperacute neurointerventional pathway and should not be reduced to the routine 14-day CEA algorithm.

Key points

  • Treat any ongoing or recurrent focal deficit through the hyperacute stroke pathway first; carotid timing never supersedes brain imaging or reperfusion assessment.
  • For a stable TIA or non-disabling stroke with indicated 50–99% NASCET stenosis, perform CEA as soon as safely feasible, preferably within 14 days of symptom onset.
  • Defer CEA or CAS after disabling stroke, infarction exceeding one third of the ipsilateral MCA territory, altered consciousness or intracranial haemorrhage until neurological and imaging reassessment supports safety.
  • Within the first 14 days, CEA is generally preferred over transfemoral carotid stenting because randomised evidence shows lower peri-procedural stroke or death.
  • CEA within the first 48 hours remains a specialist case decision: uncomplicated TIA may differ from acute infarction, and haemorrhagic transformation risk must be weighed.
  • After thrombolysis, antiplatelet and heparin treatment is withheld for 24 hours; ESVS advises considering a six-day delay before CEA or CAS.
  • Do not delay urgent referral while optimising every comorbidity; investigate and treat surgical risk in parallel and document any reason for delay.
  • Crescendo TIA or stroke in evolution can justify very urgent CEA in selected patients after multidisciplinary imaging review, rather than an automatic rule.

Ideal early candidate

Neurologically stable TIA or non-disabling stroke, confirmed ipsilateral 50–99% NASCET stenosis, limited infarct and acceptable procedural risk support early CEA.

Disabling infarctRed flag

Dense persistent deficit with modified Rankin score at least 3 suggests lower immediate benefit and higher haemorrhagic risk.

Large infarct burdenRed flag

Infarction exceeding one third of the ipsilateral MCA territory supports deferral until radiological and neurological improvement.

Crescendo symptomsRed flag

Repeated carotid-territory TIAs or stroke in evolution despite treatment require urgent multidisciplinary assessment for possible CEA within 24 hours.

After thrombolysis

Recent alteplase or tenecteplase exposure changes antithrombotic and procedural timing because intracranial and surgical bleeding risks rise.

Delayed presentation

Benefit declines with elapsed time, but a patient presenting after 14 days still needs expedited individual assessment rather than abandonment.

Red flags requiring action

  • Modified Rankin score at least 3, infarction exceeding one third of the ipsilateral MCA territory or altered consciousness supports deferring carotid intervention.
  • Stroke in evolution or crescendo TIAs despite treatment require urgent joint specialist review and may justify CEA within 24 hours in selected patients.
  • Recent intravenous thrombolysis increases bleeding risk; withhold antiplatelet and heparin treatment for the first 24 hours and consider delaying carotid intervention by six days.

Reduces early recurrent thrombosis while urgent imaging and carotid surgery assessment proceed in parallel.

Aspirin

For suspected TIA give 300 mg orally once daily immediately unless contraindicated; after ischaemic stroke give 300 mg within 24 hours once haemorrhage is excluded, continuing to 14 days or earlier definitive antithrombotic review.

Confirm swallowing route and review active bleeding, hypersensitivity, severe aspirin dyspepsia, anticoagulation, thrombocytopenia and reperfusion treatment. The operating team must specify perioperative continuation or substitution; do not create delay through an unowned antiplatelet plan.

Can reduce early recurrent non-cardioembolic stroke, but surgery and reperfusion timing require coordinated prescribing.

Clopidogrel

Within 24 hours after an eligible minor ischaemic stroke (NIHSS ≤3) or moderate-to-high-risk TIA (ABCD2 ≥4), the selected UK product uses an oral clopidogrel loading dose of 300 mg, followed by 75 mg daily. Combine with aspirin 75–100 mg daily for 21 days before continuing single antiplatelet therapy. These product eligibility criteria never justify delaying suspected-TIA referral; a named specialist protocol must govern antiplatelets when CEA is contemplated.

The selected SmPC does not recommend DAPT when CEA, thrombectomy, thrombolysis or anticoagulation is planned. Avoid in active bleeding or severe hepatic impairment; review other bleeding medicines and CYP2C19 inhibitors, especially omeprazole and esomeprazole.

06Complications, monitoring and follow-upAdverse effects, response and longer-term review.
  • Track onset-to-referral, referral-to-imaging and onset-to-CEA intervals, with a documented reason for every delay beyond 14 days.
  • Repeat the neurological examination whenever symptoms change and immediately before intervention; a new deficit returns the patient to acute stroke assessment.
  • Review brain imaging after neurological deterioration or thrombolysis before restarting antithrombotics and before carotid intervention.
  • Monitor perioperative blood pressure and headache, seizure or focal deficit because hyperperfusion and intracranial haemorrhage require urgent imaging.
  • Audit 30-day stroke and death outcomes; the procedural risk must remain low enough for preventive benefit to be credible.

Start the clock correctly

Use the most recent ipsilateral retinal or cerebral symptom onset. Starting from referral or scan date hides pathway delay and overstates timeliness.

Fourteen days is a ceiling

Preferably within 14 days means prompt safe surgery, not booking routinely on day 14. Earlier benefit is balanced against infarct and haemorrhagic risk.

Forty-eight hours is nuanced

CEA after uncomplicated TIA may be safer than after acute infarction. Very early surgery therefore requires brain imaging and an experienced joint team.

Thrombolysis changes the sequence

Withhold heparin and antiplatelets for 24 hours after intravenous thrombolysis, then restart after the required reassessment; consider a six-day procedural delay under ESVS guidance.

Deferral needs ownership

A safe decision to defer becomes unsafe if no repeat imaging, clinical review or final decision is scheduled. Name the clinician and date responsible.
