Core biopsy, fine-needle aspiration and cytology

Develop the ability to choose and interpret breast needle sampling by the question being asked, distinguishing histological architecture from cytology and checking adequacy and concordance, while explaining uncertainty and arranging reliable review.

Sampling method follows both target visibility and the information required. Core biopsy preserves architecture and can provide invasion, grade and receptor information; fine-needle aspiration supplies cells and may answer selected cyst or nodal questions. Ultrasound, mammographic or MRI guidance must be the method that actually displays the lesion, with specimen radiography used to prove calcification retrieval.

The pathology label applies only to the tissue obtained. Adequacy, needle position, marker location and radiological-pathological concordance determine whether it answers the case. Suspected lymphoma is a separate architecture-dependent pathway: NICE advises considering excision biopsy first, reserving core for situations where surgical risk outweighs benefit, and offering feasible excision after a nondiagnostic core rather than simply repeating another core.

Core preserves architecture

Core biopsy provides tissue structure for distinguishing invasive from in-situ processes in many settings and enables receptor testing when sufficient malignant material is present.

Cytology examines cells

Fine-needle aspiration can sample cyst contents or lymph nodes and may be useful in selected pathways, but isolated cells give less architectural information than a core.

Image guidance improves targeting

Ultrasound, stereotactic or MRI guidance should match lesion visibility; the specimen and marker position must correspond to the abnormality intended for diagnosis.

Categories need context

B and C categories communicate pathological certainty and adequacy, yet their clinical meaning depends on imaging suspicion, representative sampling and multidisciplinary concordance.

Lymphoma requires planned architecture

Cytology alone is inadequate for many lymphoma classifications. NG52 considers excision first; a core is an alternative when operative risk outweighs benefit and must be handled for the anticipated studies.

02Sampling selectionChoose tissue method by lesion visibilityAn abnormality requires pathological diagnosis, but it is not visible on every imaging modality.

03Discordant sample pathwayDo not let a target miss become reassurancePost-procedure review suggests that benign tissue came from beside rather than within the suspicious lesion.
  1. 1Retain the original imaging concern and document why the specimen is non-representative, including needle position or absent calcification in the cores.
  2. 2Discuss a repeat image-guided approach capable of reaching the target, such as stereotactic or vacuum-assisted sampling for mammographic-only disease.
  3. 3Explain to the patient that benign tissue was found but does not yet account for the abnormal image; avoid describing the overall result simply as benign.

  • Check puncture-site bleeding and provide advice on compression, bruising, infection symptoms and when anticoagulant management should be revisited.
  • For calcification biopsy, document whether specimen radiography demonstrated the target and whether the marker lies at the sampled site.
  • Track inadequate, indeterminate and B3 diagnoses to the specified repeat biopsy, vacuum-assisted excision or surgical decision.
  • Ensure receptor or ancillary test sufficiency is reviewed before treatment discussion when malignant tissue is found.

More tissue is not always more representative

A larger sample taken from the wrong coordinates remains non-diagnostic. Target verification matters at least as much as needle gauge or number of cores.

Cyst aspiration has a separate purpose

Aspiration may relieve a symptomatic simple cyst and confirm fluid, but a residual mass, bloody aspirate or suspicious imaging needs further assessment rather than repeated blind drainage.

Lymphoma changes the tissue plan

Core preserves more architecture than cytology, but NG52 still advises considering excision first for suspected non-Hodgkin lymphoma. If a risk-justified core is nondiagnostic, offer feasible excision rather than simply repeating core.

Categories are communications tools

B1 to B5 and C1 to C5 convey adequacy and diagnostic concern within their sampling systems. They do not independently prove imaging-pathology concordance.

07Common pitfallsFrequent interpretation and management errors.
  1. 01

    Using fine-needle cytology alone to establish a non-operative screening cancer diagnosis omits tissue architecture and conflicts with current NHS England guidance.

  2. 02

    Calling scant normal tissue benign when the target was missed converts an inadequate procedure into false reassurance.

  3. 03

    Sampling a mammographic-only lesion freehand under ultrasound cannot demonstrate that the intended abnormality entered the specimen.

  4. 04

    Stopping after a B3 result without lesion-specific multidisciplinary planning ignores heterogeneous upgrade risk and incomplete sampling.

    Treating core as the automatic first procedure for suspected lymphoma overlooks NG52 preference for excision and risks another inadequate sample when a nondiagnostic core is merely repeated.
