Synopsis

Develop the ability to choose and interpret breast needle sampling by the question being asked, distinguishing histological architecture from cytology and checking adequacy and concordance, while explaining uncertainty and arranging reliable review.

  • Core biopsy removes tissue cylinders, preserving architecture needed for many breast diagnoses and often providing material for grade and receptor testing.
  • Fine-needle aspiration yields cells rather than tissue architecture; it can answer selected cyst or nodal questions but an inadequate aspirate does not exclude malignancy.
  • NHS England assessment guidance states that cytology should not be used alone to make a non-operative breast-cancer diagnosis in screening assessment.

Mammography shows clustered pleomorphic calcification without an ultrasound correlate. A freehand fine-needle aspirate yields scant benign ductal cells.

  1. Identify that calcification, rather than a palpable mass, is the target and select mammographic image guidance capable of proving that the sampled material contains it.
  2. Arrange stereotactic or tomosynthesis-guided tissue sampling with specimen radiography, using core or vacuum-assisted technique according to the assessment protocol.
  3. Treat the scant benign cytology as non-diagnostic for the suspicious calcification because it neither preserves architecture nor demonstrates target retrieval.
  4. Review histology, calcification retrieval and post-biopsy marker position together, escalating repeat or surgical sampling if the result remains discordant.
  5. Verify that pathology has adequate material for the required classification and that the patient receives one integrated result rather than competing test reports.
