Liver, pancreas and renal lesion characterisation

Characterise focal liver, pancreatic and renal lesions with organ-specific multiphasic imaging, separate simple benign appearances from indeterminate or malignant features, and direct each lesion to the correct specialist pathway.

Complicated focal lesion

Haemorrhage, rupture, biliary or urinary obstruction, pancreatitis, sepsis or vascular invasion may convert an incidental lesion into urgent disease.

Action: Stabilise, identify the complication with an appropriate phase or modality and involve hepatobiliary, pancreatic, urological or interventional specialists promptly.

01Core principlesThe concepts and mechanisms needed to understand the subject.

A focal lesion discovered on a single portal-venous CT is often incompletely characterised. The reader should describe organ of origin, size, margins, attenuation or signal, calcification, fat, enhancement, duct or collecting-system relation, vascular involvement and comparison. A recommendation should name the modality and question rather than simply say follow-up.

Liver assessment changes with background risk. A classic benign pattern in a healthy liver may need no further action, while cirrhosis or known malignancy raises pre-test probability and invokes disease-specific multiphasic criteria. MRI offers tissue and hepatobiliary contrast; multiphasic CT is fast and stages suspected malignancy. Biopsy is considered only after imaging and MDT review because vascular lesions, sampling error and tract consequences matter.

Pancreatic lesions include solid masses, inflammatory collections and cystic neoplasms. Pancreatic-protocol CT assesses enhancement, vessels and metastases; MRI-MRCP depicts duct communication and internal cyst features; EUS can inspect mural nodules and obtain tissue selectively. Renal characterisation focuses on true enhancement, cyst complexity, macroscopic fat and extension.

Key points

  • Characterisation asks whether a lesion contains fat, fluid, blood or enhancing tissue and how it behaves across phases; size alone rarely answers this.
  • First identify organ, patient cancer and liver-risk context, symptoms, prior imaging and whether the study was adequately protocoled.
  • Use contrast-enhanced MRI preferentially when a benign liver lesion is suspected, while CT provides rapid assessment when malignancy or complications are concerns.
  • For possible adult RCC, NICE NG256 recommends multiphasic contrast-enhanced abdominal CT, or MRI when CT is unsuitable.
  • For pancreatic cysts, use pancreatic-protocol CT or MRI-MRCP; consider EUS when malignancy likelihood or need for surgery remains unclear.
  • Do not invent a universal follow-up interval: organ, lesion class, size, growth, patient risk and definitive guideline determine action.

True enhancement

An increase in lesion attenuation or signal after contrast indicates vascularised tissue; compare matched phases and account for pseudoenhancement.

Simple cyst pattern

Thin wall, homogeneous fluid signal, no septum, nodule or enhancement supports a benign simple cyst in the appropriate organ.

High-risk context

Cirrhosis, known malignancy, hereditary syndrome, pancreatitis history or visible haematuria changes prior probability and the correct algorithm.

Pancreatic warningRed flag

Solid component, mural nodule, main-duct change, obstructive jaundice or upstream atrophy raises concern for malignant or pre-malignant disease.

Renal invasionRed flag

Renal sinus or perinephric extension, renal-vein thrombus, nodes and distant disease affect staging and treatment planning.

Lesion complicationRed flag

Rupture, haemorrhage, infection, duct obstruction or organ dysfunction requires acute assessment separate from long-term neoplastic risk.

Red flags requiring action

  • Hypotension, falling haemoglobin or severe pain with a liver or renal mass suggests haemorrhage and requires urgent multiphase assessment.
  • Obstructive jaundice, pancreatic-duct dilatation, upstream atrophy or a solid pancreatic component requires expedited specialist evaluation.
  • Enhancing soft tissue in a cystic renal lesion, renal-vein extension or metastatic features raises RCC concern and needs uro-oncology review.
  • A new liver lesion in cirrhosis or known cancer follows a high-risk pathway rather than a generic incidental-lesion algorithm.

Incidental is not benign

Incidental describes how a lesion was found; morphology, patient risk and interval behavior determine its significance.

Phase creates appearance

A lesion can be inconspicuous or mimic another diagnosis if arterial, portal or delayed enhancement is not compared appropriately.

Fat needs classification

Macroscopic fat can support specific benign lesions, while microscopic fat and mixed tissue require organ-specific interpretation.

Bosniak is cystic

Bosniak classification applies to cystic renal masses after adequate enhancement assessment and should not be used for every solid lesion.

Biopsy has a question

Sampling is useful when histology changes management; it can be nondiagnostic and should not precede adequate imaging by reflex.

Surveillance is individualised

A follow-up interval should cite the applicable lesion pathway and account for age, comorbidity, growth and treatment candidacy.
