A vestibular schwannoma is a usually slow-growing benign nerve-sheath tumour arising from the vestibular portion of the eighth cranial nerve. The older term acoustic neuroma remains common, but it is less anatomically precise. A lesion may begin within the internal auditory canal and extend into the cerebellopontine angle, where proximity to the cochlear, facial and trigeminal nerves explains its clinical effects. Benign histology does not mean that growth can never threaten function.
Many patients present with gradually asymmetric hearing or persistent unilateral tinnitus rather than dramatic vertigo. Slow vestibular change can be partly compensated, leaving subtle imbalance. Some lesions are incidental. Management is therefore individualised: a small stable lesion with manageable symptoms may be monitored, whereas growth, brainstem compression or other features may favour intervention. The choice should consider hearing in both ears, age, comorbidity, lesion size and behaviour, treatment risks and the person's preferences.

Sporadic nerve-sheath tumour
Most vestibular schwannomas occur as isolated tumours without an identified inherited syndrome. They arise from Schwann cells supporting the vestibular nerve.
NF2-related schwannomatosis
An inherited or mosaic tumour-predisposition condition can produce bilateral vestibular schwannomas and other nervous-system tumours, with implications for broader surveillance and family counselling.

Growth within a confined bony canal can affect neighbouring cochlear and facial nerve structures, producing symptoms despite a relatively small overall tumour volume.

Slow change in vestibular input may allow partial central compensation, so imbalance can be more prominent than the dramatic spinning of an acute vestibular injury.

A larger lesion may contact the brainstem or adjacent cranial nerves and disturb cerebrospinal fluid pathways, producing neurological effects beyond hearing and balance.

Ask about telephone use, speech understanding in noise and whether one ear has become consistently less useful. Hearing loss is commonly gradual but can be noticed suddenly. Otoscopy may be normal, and an apparently mild audiogram does not exclude a clinically important localising symptom.
Persistent sound in one ear deserves assessment, especially with asymmetric hearing. Most unilateral tinnitus is not caused by a tumour, so explain the purpose of investigation proportionately. The absence of tinnitus does not exclude a schwannoma.
Patients may report veering, insecurity in darkness or difficulty on uneven ground. Severe recurrent rotational attacks can have other causes and should not be automatically assigned to an incidental small lesion. Balance assessment helps identify additional treatable contributors.
Facial numbness, altered corneal sensation, facial weakness or other cranial nerve findings may indicate a larger or differently positioned lesion. Examine rather than relying only on the patient's description, and distinguish an acute deficit requiring emergency assessment from a stable chronic finding.
Headache, vomiting and progressive gait impairment can indicate mass effect or hydrocephalus. Bilateral vestibular tumours, young presentation, multiple schwannomas or relevant family history raise the possibility of an inherited tumour-predisposition syndrome and merit specialist genetic assessment.

- NICE recommends offering MRI to adults with hearing loss and localising symptoms or signs, such as facial nerve weakness, regardless of pure-tone thresholds. Specialist radiology determines the imaging protocol and contrast requirements; the request should state the clinical concern clearly.

Age or noise-related hearing loss
Common cochlear causes may explain hearing impairment, but marked asymmetry or localising signs require assessment rather than attribution to age or exposure alone.
Ménière disease
Fluctuating auditory symptoms with spontaneous vertigo attacks suggest an episodic inner-ear syndrome. MRI may still be indicated when asymmetry requires a retrocochlear explanation to be excluded.
Other cerebellopontine-angle lesions
Meningioma and other structural lesions can cause overlapping cranial nerve symptoms. MRI anatomy and specialist interpretation distinguish these from a vestibular nerve-sheath tumour.

Hearing and communication loss
Progressive or treatment-related auditory impairment can affect conversation, localisation and employment, making audiological rehabilitation a central component of long-term care.
Facial weakness and corneal exposure
Tumour effects or treatment can impair facial nerve function, reducing blink and eye closure. Corneal protection requires active assessment rather than reliance on reported dryness.
Brainstem compression or hydrocephalus
Substantial growth can compromise adjacent structures or cerebrospinal fluid circulation, producing progressive neurological symptoms and potentially requiring urgent neurosurgical management.

- Ensure surveillance imaging is booked and reviewed by the responsible team; a scan without communicated interpretation is incomplete follow-up.
- Document hearing in both ears and discuss assistive devices, hearing aids or specialist implant assessment according to residual function and the individual situation.
- Review facial movement, ocular comfort and closure after intervention; a painful or red exposed eye warrants prompt ophthalmic assessment.

Radiological control and function
Stopping growth does not reliably restore established hearing loss. Discuss tumour control and functional goals separately so that the person does not expect radiation or surgery to reverse every symptom.
Observation is purposeful care
For a small lesion without threatening mass effect, treatment risks may exceed the immediate benefit of intervention. Serial assessment allows change to be detected while avoiding unnecessary procedural harm, provided follow-up remains reliable.
The only useful hearing ear
Treatment choices become especially sensitive when the opposite ear has major hearing impairment or a bilateral tumour syndrome is possible. Specialist discussion should include the consequences of losing residual hearing and the available rehabilitation options.
Symptoms after treatment
Transient swelling after radiation or postoperative vestibular change can complicate interpretation of new symptoms. Patients need direct contact instructions and a review pathway; they should not simply wait for the next routine scan if deterioration is substantial.
